The DNA LifePrint Management Kit has been designed to be the most effective system for
collecting and retaining a DNA sample. The following illustrates how the collection can be
done in six simple steps.

Insert vial into DNA Vial Holder located on back of box	Place cotton swab into vial. Repeat step 3 & 4 with other cotton swab
Remove cap from vial. Remove cotton swabs from packaging	Replace cap securely . (Important: Cotton swabs must remain inside closed vial)
Rub cotton swab against inside cheek for 5 seconds. (Cotton swab now contains DNA sample)	Complete the enclosed red envelope and place securely closed vial into the envelope
	Store envelope in a safe place at room temperature. The envelope may also be placed back into the DNA LifePrint box for added security
